Coach Resume

Must be filled out by ALL COACHES and faxed to Cooperstown Dreams Park (704-630-0737) by April 1, 2008!

Agreement to Participate with Waiver
and Release of Liability Form

Must be filled out by ALL COACHES. Parents must sign if coaches are not 18 or above! Forms are to be turned in by Team Contact at Registration!

Camp Health History and Examination Form

All COACHES must complete their Camp Health History and Examination Form but a doctor's signature is NOT required (unless coach is under 18 years of age). Forms are to be turned in by Team Contact at Registration!
